Insurance Summary

Health Insurance:

Blue Cross/Blue Shield-PPO (Includes coverage out-of-state.)
Single Coverage: Paid in full by SS&A.
Family Coverage: SS&A pays approximately 80% of cost.
PMD Doctor co-pay is $20.00. Major medical and prescriptions are reimbursed at 80% after an annual deductible is satisfied. Generic drug prescriptions are reimbursed 100% without any deductible to satisfy. Additional insurance is made available to the employee through AFLAC, at the employee's expense.

Dental Insurance:

Southland National Insurance.
Two options are available from which to choose:
The basic plan is paid in full for the employee by SS&A. Family coverage is an option available at the employee's expense. The basic plan covers preventive checkups and x-rays at 100%. Other basic services are covered at 50%, after meeting a $50 annual deductible.
The high option, for which the employee pays the difference in cost above the low option, covers basic services at 80%, with preventive services and deductibles the same as the basic plan, and with an addition of major services covered at 50%. Orthodontia, for dependents under age 19, is covered at 50% up to $1000 lifetime maximum under the high option plan.

Vision Care:

Yearly exams and lenses, with biannual frame coverage is available under this plan, with a small co-pay. Employee pays for this coverage.

Long-Term Disability:

Paid in full by SS&A. After 90 days disability, policy will pay 2/3 of salary, up to $5,000 per month.

Life Insurance:

Employee is covered by a $50,000 Term Policy, paid for by SS&A, that will double to $100,000 coverage in case of accidental death.
